Key Responsibilities

The Community Services Support Specialist will:

  1. Collaborate as part of the North Area Community Services Team to deliver crisis intervention, assessment, and case management support to residents facing housing instability.
  2. Conduct vulnerability assessments for homeless individuals and families in accordance with established standards and policies.
  3. Oversee HMIS data for all Community Services Programs and assist with training and data accuracy.
  4. Perform additional tasks as assigned within the scope of this role.

Essential Duties

  1. Provide support to Community Services Programs to facilitate casework for families and individuals receiving assistance.
  2. Evaluate individuals and families experiencing homelessness or at risk of homelessness for housing-related services, entering eligible clients into the Coordinated Entry System.
  3. Organize educational workshops and series for community members and clients of Community Services.
  4. Implement a variety of support services tailored to client needs, including transportation, SOAR applications, and connections to community resources.
  5. Assess and manage cases for individuals with mental health, emotional, or substance abuse challenges within Supportive Housing Programs.
  6. Work in partnership with case managers and vocational counselors to coordinate services based on the needs of participants in the SHIFT Program.
  7. Ensure adherence to McKinney-Vento laws regarding the education of homeless students and facilitate access to services for these students.
  8. Complete necessary documentation for services rendered or referrals made, as directed by the Assistant Director of Community Services.
  9. Review client records to ensure compliance with HUD, accrediting bodies, and Catholic Charities' Performance Quality Improvement Plan.
  10. Manage the distribution and analysis of client satisfaction surveys for the Community Services North Division.
  11. Screen and conduct intake assessments for client eligibility for various assistance programs, consulting with Case Managers as necessary.
  12. Collaborate with the Assistant Director of Community Services and other staff to ensure that the needs of families seeking transitional and permanent supportive housing are met with respect and dignity.

Qualifications:
  1. A Bachelor's degree in a relevant field is required, with a preference for human services.
  2. At least one year of experience in human services is necessary.
  3. Experience working with families in crisis is preferred.
  4. Must obtain and maintain SOAR (SSI/SSDI Outreach, Access and Recovery) credentials.
  5. Flexibility to work varying days and hours as needed.
  6. Understanding of program operational boundaries is essential.
  7. Ability to work effectively with diverse populations and backgrounds.
  8. Strong verbal and written communication skills are required.
  9. Completion of background checks and any required testing is mandatory.
  10. A valid driver's license and reliable transportation are necessary.
