We are seeking an outgoing, empathetic, organized, and self-motivated individual for the Care Manager position. The goal of the Care Manager is to engage, connect, and remotely monitor patients with various health conditions. Simply, this job is about understanding and tracking patient symptoms in order to better facilitate care between the patient and their provider. You will be communicating with patients on a daily basis using pre-built questionnaires, assessments, and screenings. The relationships you build will be rewarding, impactful, and ultimately will advance the standard of their care.

Responsibilities:

  • Engage and connect with patients on a daily basis
  • Administer pre-built questionnaires and health assessments
  • Maintain awareness and education on various patient conditions
  • Work with other Care Managers to best coordinate care between patients and their provider

Minimum Credentials:

  • Medical Assistant (MA)

Preferred Qualifications:

  • Certification in Case Management (CCM, ACM) upon hire
  • 1 year of Clinical experience in an HMO, medical group, affiliated model, hospital or medical office/clinic setting
  • 1 year of experience in utilization management or case management

Competitive Pay ($11.00 - $14.00USD / hour)
