This is temporary for a few months, can be full or part time.  Work is done in person, mostly at 1 site but 1 day/ week is community based.  Hours are M - F basic business hours.

The program serves clients 55+ with complex medical and psychosocial problems. Social Workers are in a key role providing biopsychosocial assessments, care coordination, and case management services for a client caseload in collaboration with a dynamic interdisciplinary team. The Social Worker supports client self-determination and promotes client-centered care while engaging in development of behavioral and other health interventions to enhance quality of life in a center-based, geriatric primary care setting.

 

Qualifications:

  • MSW
  • 1 year experience working with disabled adults and older adults with mental and/or behavioral health diagnoses and substance abuse disorders
