Location : Philadelphia, PA 19140
Project Duration : 3+Months (Contract)
Pay rate : $33/hr.
Job Description:
Hours: Days 8a-430p
Social Worker will work remote and travel to personal care homes/facilities.
Job Summary
- The Social Worker assesses the patient's psychological situation after acute or post-acute discharge through telephonic outreach and home visits in order to make recommendations to medical personnel, and provides direct services that ensure the patient's best use of health care services that focus on readmission prevention and supports patient's functional status within their home or post-acute care setting.
Requirements:
- Education Master’s Degree : Accredited School of Social Work (Required)
- Combination of relevant education and experience may be considered in lieu of degree.
- Experience 2 Years’ experience as a Social Worker in an ambulatory or community setting. (Required)
- 2 Years’ experience as a Social Worker in a hospital setting. (Preferred)
- License/Certifications CPR - Cardiopulmonary Resuscitation (Preferred)
- SW-LIC - PA Social Worker License (Preferred)
