We are seeking a skilled Social Worker to join our team at Social Work PRN in Philadelphia. As a per-diem, temp to hire position, you will have the opportunity to develop and implement social support services for an inpatient hospital and provide comprehensive patient support.
Key Responsibilities:- Conduct psychosocial assessments to identify patient needs and develop effective support plans.
- Manage high-risk patients and develop discharge plans to ensure a smooth transition to community care.
- Collaborate with multidisciplinary teams to address psychosocial needs related to vision care.
- Evaluate barriers to care and improve access to health resources for underserved communities.
- Maintain a resource directory for the visually impaired and provide support throughout surgical phases.
- Document patient information for CDC studies and complete assessments as required.
- Collaborate with the research director on manuscripts and contribute to the development of new research initiatives.
- PA LSW or LCSW licensure
- Experience working with older adults, mental health, and underserved communities
- At least 10 years of experience in healthcare, with a focus on vision-related social work and research
