Responsibilities:

  • Oversee medical and health service activities and personnel
  • Plan and coordinate health services
  • Work collaboratively with physicians, care team and care partners to provide appropriate services to clients
  • Enforce healthcare policies and procedures resulting in excellent client advocacy

​Qualifications:

  • Previous experience in healthcare and case management
  • Strong persuasive, caring and empathetic qualities
  • Strong organizational, written and verbal communication skills
  • Ideal candidate has a RN or MSW. Must have clinical license or certification: RN/LPN/MSW/CSW/CCM/CMC or comparable experience
