Job DescriptionJob DescriptionPURPOSE AND SUMMARY:
Under the direction of the Owner and Director, the Caregiver Manager is the primary person responsible for assisting clients and their support systems in maintaining the highest possible level of independence and functioning in an apartment unit style community setting. This role involves comprehensive oversight of caregiver staff, ensuring that service delivery meets the highest standards of care in a vertically integrated manner.
DUTIES AND RESPONSIBILITIES:
1. Caregiver Oversight and Collaboration:
  • Manage and supervise caregiver staff across 12-hour shifts (8:00 AM to 8:00 PM or 8:00 PM to 8:00 AM).

  • Assign tasks, delegate responsibilities, and ensure proper shift rotation and handover summaries.

  • Collaborate with caregivers to maintain a harmonious and efficient work environment, promoting teamwork and effective communication.

  • Resolve conflicts that arise between coworkers to maintain a positive work environment. In addition, they foster a spirit of teamwork and cooperation to create unified teams.

  • Training all oncoming staff with the facilities “IN-UNIT” training checklist.


2. Medication Management:
  • Oversee the organization and maintenance of medication closets for the duration of each shift.

  • Ensure proper documentation of pill passes, confirming whether residents have taken their medications or if there are discrepancies.

  • Address any medication-related issues promptly and accurately.

3. Resident De-escalation and Crisis Management:
  • Assessing the needs of residents in the facility who have difficulty coping in the community, this includes: deescalating ongoing behaviors, redirecting behaviors to promote positive behavior, and callaborating with the resident’s care team icluding case managers, nurses, doctors and gaurdians.


  • Act as the designated person for intervening when caregivers are unsuccessful in deescalating residents.

  • Implement appropriate strategies to handle and resolve conflicts, ensuring the safety and well-being of residents and staff.


4. Documentation and Compliance:
  • Ensure that caregivers adhere to documentation protocols, maintaining accurate and comprehensive records of all care provided.

  • Uphold compliance with agency policies, procedures, and applicable health regulations.

5. Uniform and Professionalism:
  • Ensure that all caregivers are in proper uniform and present themselves professionally at all times.

  • Conduct regular checks to enforce dress code and hygiene standards.

6. Service Planning and Coordination:
  • Monitor and reassess client needs with regard to each clients ISP (individual service plans) to encourage optimal client independence and participation.

  • Liaise with service providers to arrange necessary services, equipment, and transportation.

7. Quality Assurance and Continuous Improvement:
  • Support and orient professional and paraprofessional staff to ensure the effective coordination and delivery of client-focused services.

  • Promote continuous quality improvement and assist with program planning, resource utilization, and risk management.

8. Community and Client Relations:
  • Ability to accompany residents on outtings, doctor’s appointments and errands as needed


QUALIFICATIONS:
Education:
  • C.B.R.F. Certification

Experience:
  • Minimum of 18 months’ “exemplary-experience,” in home care service delivery or coordination.

  • Proven ability of working with clients with various behavioral/mental health diagnosis such as, autism, schizophrenia, dementia and explosive mood disorder, just to name a few.  

Skills/Abilities:
  • Strong leadership and organizational skills.

  • Excellent interpersonal, written, and verbal communication skills.

  • Proficiency in computer applications, especially assisted living software, such as ECP.

  • Ability to exercise discretion with confidential information, in compliance with HIPPA guidelines.

  • Knowledge of community based healthcare dynamics and resource networks.


CONDITIONS OF EMPLOYMENT:
  • Must be able to work flexible hours and provide on-call services.

  • CBRF certification
