We are seeking a full-time Family Nurse Practitioner (FNP)

About Our Organization and Mission

Dakota Child and Family Clinic is a nonprofit community health organization located in Burnsville, Minnesota. Our primary care services are based on the Nurse Practitioner framework of patient-centered, high-quality, holistic and culturally inclusive care. Nurse practitioners focus on helping patients achieve optimum health, however that is defined by the individual. We were the first nurse practitioner based clinic in Minnesota.

We have a strong commitment to ensuring that all members of our community have access to the care and services they need to lead healthy lives. We offer a sliding fee scale and will not turn patients away due to inability to pay. Our work focuses on meeting the needs of underserved communities, including (but not limited to) BIPOC, LGBTQIA+, low-income and neurodivergent community members. A meaningful number of our patients are newer to the U.S. and English may not be their primary language.

Our primary care services fit within a broader framework of programming at the clinic which includes community-based healthcare, wraparound supports, support groups, community outreach and educating future healthcare workers.

The ideal candidate will be committed to our mission, comfortable working with a wide range of communities, be ready to meet our patients where they are and work in partnership with them to achieve optimal health, however the patient defines that.

 

Hours vary, clinic is open M-Th 8-5, F 8-12 and Sat 8-3

 

Overview of Nurse Practitioner Position

Nurse practitioners (NP) assume responsibility and accountability for the provision of independent primary care in the clinic setting to a specified population of patients. The NP: 1) provides 24-hour service by appointment, telephone consultation, and after-hours answering services; 2) serves as preceptor and clinical instructor for graduate nursing students; 3) provides consultation and advice to others as a highly specialized practitioner; and 5) participates in nursing and health related research as is feasible.

Job Responsibilities

  • Provides anticipatory guidance, screening and health education to patients for the promotion and maintenance of health

  • Initiates treatment for patients with commonly occurring health problems which lie within the nurse specialist’s scope of practice by using a systematic approach to history and physical appraisal of patients and/or families through clinical judgment of evaluated data. Prescribes, coordinates, delegates and evaluates care.

  • Prescribes medication appropriate to the patients’ health conditions.

  • Monitors patients with long-term or chronic illnesses and facilitates adaptation responses of these individuals and their families to the illnesses.

  • Plans appropriate care utilizing community resources and acts as a liaison by relaying necessary information to other health care professionals and agencies for the provision of care continuity.

  • Documents all patient information and plans of care in a timely manner as required to advance continuity of care

  • Educates individuals and/or families or groups about health or about specific defined health problems.

  • Participates in community outreach activities as assigned

  • Participates and/or demonstrates leadership in professional activities such as quality assurance programs, research projects and/or the education of other staff and graduate nursing students within the DCFC clinic.

  • Maintains current and evidence-based knowledge and practice through independent reading and attendance at professional educational offerings.

  • Demonstrates leadership and ongoing professional development by maintaining licensure and credentials in good standing, pursuing opportunities to present clinical improvements, and maintains prescriptive authority.

  • Promotes guest relations; anticipates and responds to the needs of others; provides assistance in a courteous, quick manner; treats others with care and respect while maintaining privacy, confidentiality, and dignity.

  • Maintains a professional appearance and a clean, safe environment.

  • Providers are expected to rotate on call and weekend shifts.

 

 

Minimum Qualifications and Level of Education

State of Minnesota current independent Advanced Practice Registered Nurse licensure

Master’s Degree in Nursing with Nurse Practitioner focus with advanced skills in physical assessment, pharmacology and pathophysiology.

Current Board Certification in area of Specialty practice

  • Pediatrics: Pediatric Nursing Certification Board (PNCB) or American Nurses Credentialing Center (ANCC)

  • Adult: American Academy of Nurse Practitioners Certification Program (AANP) or American Nurses Credentialing Center (ANCC)

  • Family: American Academy of Nurse Practitioners Certification Program (AANP) or American Nurses Credentialing Center (ANCC)

  • Women’s Health: National Certification Corporation for the Obstetric, Gynecologic and Neonatal Nursing Specialties (NCC)

  • Psychiatric Mental Health: American Academy of Nurse Practitioners Certification Program (AANP) or American Nurses Credentialing Center (ANCC)

Minimum of 2080 hours of professional nurse practitioner experience required
