This is an exempt position that reports directly to the Manager of Pharmacy Operations. The role of the Pharmacist position is to assist DeliverCareRx in acquiring, dispensing patient medications and servicing hospice inpatient units, while ensuring medication therapy is appropriate for the patient.

Key elements of the role include:

  1. Primarily collaborate with co-workers and other healthcare professionals to acquire, review prescriptions for hospice inpatients to determine appropriate medication therapy.
  2. Daily review and verification of physician medication orders and patient profiles
  3. Ensure hospices are in compliance with state/federal automated dispensing system (ADS) and prescription regulations
  4. Assist in troubleshooting with ADS issues
  5. Prepare/order stock medications for the automated dispensing systems (Pyxis or Omnicell), while closely monitoring par levels and utilization of medications
  6. Provide reports to inpatient units as requested
  7. Assist with daily workflow for home based patients
  8. Oversee medications prepared by pharmacy technicians
  9. Provide Drug Utilization Review (DUR)
  10. Focus on quality assurance to correctly interpret physician orders and to prevent medication errors
  11. Provide high quality customer service to patients, caregivers, healthcare providers
  12. Counsel patients and caregivers on medication therapy as needed
  13. Utilize sound judgment to make appropriate decisions in the best interest of patients and the company
  14. Provide rotating on call services to patients on an emergency basis
  15. Assist with inventory functions including, but not limited to ADS and physical inventory, cycle counting, medication adjustments
  16. Travel to inpatient units in Florida as needed
  17. Other duties, as assigned

The successful candidate for this role will include the following experience and capabilities:

  1. Florida licensed pharmacist in good standing with the board of pharmacy (PharmD or BS pharmacy)
  2. Hospice and automated dispensing experience preferred
  3. Flexible work schedule to include nights and weekends
  4. Self-motivated and energetic team player with a positive attitude
  5. Highly organized and efficient in a fast paced and growing environment
  6. Working knowledge of computer technology for prescription management
  7. Ability to function independently and effectively in a self-directed environment
  8. Excellent problem solving skills
