If so, we have the perfect opportunity for you The LVN Case Manager will play a crucial role in supporting the case management process, assisting with the coordination of patient care, and ensuring that all aspects of care are communicated effectively among patients, families, and healthcare providers. This position requires excellent organizational skills, the ability to manage multiple tasks simultaneously, and a strong commitment to patient care.

Key Responsibilities:

  • Assist case managers in the development and implementation of patient care plans.

  • Coordinate appointments and communicate scheduling information to patients, families, and home health care providers.

  • Maintain accurate and up-to-date patient records and documentation in the electronic health record (EHR) system.

  • Serve as a liaison between patients, families, home health team members, and external providers to ensure seamless communication and care coordination.

  • Monitor patient progress and report any changes or concerns to the case manager.

  • Assist with the verification of patient insurance coverage and the processing of new orders and changes to the plan of care.

  • Participate in case conferences and staff meetings, providing administrative support as needed.

  • Perform administrative duties such as answering phones, managing correspondence, and ordering supplies to support the case management team.

Requirements:

  • 1-2 years of experience in a healthcare setting, with experience in home health or case management preferred.

  • Strong organizational and time management skills, with the ability to prioritize tasks effectively.

  • Excellent communication and interpersonal skills, with a compassionate approach to patient care.

  • Proficiency in Microsoft Office Suite and experience with electronic health records (EHR) systems.

  • Ability to work independently and as part of a multidisciplinary team.

  • Detail-oriented with a commitment to accuracy and confidentiality in all work.

  • LVN or RN license
