Tender Care Home Health & Hospice offers excellent career opportunities for a PRN Wound Care Nurse Practitioner who provides individualized, goal-directed care to perform homebound visits for our patients. The ideal candidate will have a passion for wound care, strong clinical skills, and the ability to work independently in a home health setting.

Wound Care Nurse Practitioner Responsibilities:
- Conduct comprehensive assessments of patients with wounds, including evaluating the wound's condition, progress, and response to treatment.
- Develop and implement individualized care plans in collaboration with patients, families, and the healthcare team.
- We believe that informed patients are empowered patients. Our program includes extensive education for patients and their families on wound care practices, lifestyle modifications, and preventive measures to enhance healing and prevent recurrence.
- Educate patients and their families on effective wound care practices and preventative measures.
- Maintain accurate and timely documentation of assessments, care plans, and progress notes in compliance with company policies and regulations.
- Communicate effectively with interdisciplinary teams to ensure coordinated care and optimal patient outcomes.
- Our program utilizes the latest evidence-based treatments and technologies, including advanced dressings, negative pressure wound therapy, bioengineered skin substitutes, and nutritional support, all designed to facilitate healing and reduce the risk of complications.
- Continuity of Care: We emphasize continuity of care, ensuring patients receive seamless support throughout their healing journey. We maintain open lines of communication with referring physicians and other healthcare providers to coordinate care effectively.
Wound Care Nurse Practitioner Skills:
- Active Nurse Practitioner license in Texas.
- Certification as a Wound Care Nurse Practitioner (CWCN) or equivalent experience preferred.
- Strong clinical assessment skills and the ability to make independent decisions.
- Excellent communication and interpersonal skills.
- Valid driver’s license and reliable transportation for home visits.
