Hospice LPN PRN Weekend On-Call Specialist

Hillcrest Health Services is seeking a compassionate and skilled Hospice LPN to provide holistic, compassionate, and skilled end of life care to our patients.
Key Responsibilities:- Assess the physical, functional, psychosocial, and nutritional status of assigned patients to provide required patient services.
- Collaborate with healthcare professionals, patients, and families to develop and implement individualized care plans.
- Provide nursing services in accordance with established standards and protocols.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in interdisciplinary team meetings to ensure coordination of patient care and services.
- Prior experience in hospice or home health care is preferred.
- LPN licensure in the state of Iowa or a compact state is required.
- Three years of experience as a nurse is required.
- Valid driver's license, current automobile insurance, and reliable transportation are necessary.
- Average to above-average computer skills are required.
- Excellent communication and interpersonal skills are essential.
