The RN Case Manager is responsible for delivering direct patient care services while effectively managing patient needs through specialized knowledge and experience.

- Conduct thorough assessments of the physical, functional, psychosocial, and nutritional status of assigned patients systematically and continuously to ensure the provision of necessary patient services.
- Collaborate professionally with physicians, team members, patients/families, and other caregivers to develop a comprehensive plan of care and coordinate service delivery.
- Administer nursing services in alignment with the established plan of care, utilizing nursing processes effectively.
- Maintain accurate records of time spent on visits, travel, meetings, mileage, etc., and enter them into the time tracking spreadsheet.
- Must hold a valid nursing license in the state of Iowa or a compact state.
- A minimum of three years of nursing experience is required.
- Preferred: At least two years of experience in a hospice or home care environment.
- A valid driver's license is necessary.
- Reliable transportation and current automobile insurance are required.
- BLS certification from the American Heart Association is mandatory.
