  • Generate recommendations for process improvements whenever appropriate
  • Participate in developing best practices, sharing resources, and ongoing trainings

 POSITION SUMMARYThe Community Health Advocate I works closely with internal and external stakeholders with primary focus on supporting community outreach and education.  DAY-TO-DAY RESPONSIBILIES
  • General program support
  • Receives training and provides outreach and education across multiple health topics based on community need
  • Supports the planning and implementation of community outreach and education efforts, including off-site location event set-up and take down 
  • Canvass neighborhoods to promote community events and to provide outreach and education and resources
  • Operates computer and smartphone to access e-mail, communications software, electronic calendars, and other basic office support software
  • Assists with special events planning
  • Works with community members to identify opportunities for referrals and resource linkages
  • Assembles outreach education materials and curriculum for use as handouts, teaching and presentations
  • Provide set-up and management of traffic flow at scheduled events
  • Informs community of outcomes as a result of screening, testing and/or vaccinations. Identifies and addresses individual concerns and provides appropriate next steps
  • Helps to acquire, implement, monitor and track data and program monitoring and evaluation methods
  • Performs other duties as assigned

  • Trustworthiness/Integrity: Ability to at all times use discretion, maintain confidentiality, protect Protected Health Information (PHI), not disclose Personal Identifiable Information (PII), and exhibit ethical conduct
  • Stress Tolerance:Ability to administer self-care and have an awareness of when personal wellness needs to be acknowledged and addressed
 EDUCATION
  • Education: High school diploma or equivalent
  • Some college/technical training preferred
 REQUIRED QUALIFICATIONS AND WORK EXPERIENCE·         Proficient in Microsoft Office Suite and related business software·         Experience conducting telephone-based or in-person interview, data collection and data entry ·         Must have consistent reliable transportation ·         Weekend and evening availability required
  • One-year experience in community outreach, health education, advocacy, linkage to care, social services, public health or related field

BWPC frequently conducts work in hospital and clinic settings and may offer on-site healthcare services, such as COVID-19 testing.  Requires frequent oral and written communication with internal and external stakeholders, vendors, and partners, the ability to enter data and written communications in electronic format in a timely manner using various software.  May be required to work at other locations within the community. Ability to consistently reach the worksite, meetings, and events on time and prepared to work. WORK METHODS·         In-person ·         Reports to: Community Health Advocate II/Community Coordinator/Human Resource Manager
 COMPENSATION AND PAY SCALENon-Exempt, hourly. Pay grade I.
