POSITION SUMMARY:

Identifies, triages and manages medically underserved patients within the integrated primary, community and school health care setting. Work within a system of interdisciplinary departmental teams and/or contract service agencies providing assessment, prevention, intervention, treatment, and related ancillary support services via an integrated service delivery system to people with alcohol and other drug-related problems, mental illness, and/or co-occurring conditions. These patients may struggle with medical conditions, mental illness, alcohol and/or drugs or chronic diseases. Services are provided through ongoing psychotherapy, psycho-education, warm hand-off approaches or community outreach. In compliance with all state statutes and regulating bodies, in accordance with KCHC Policy and Procedure and within the scope of training and skill level, assess, diagnose, counsel, and provide AODA and general psychotherapeutic treatment, rehabilitation, and care coordination for children, adolescents, and adults (individual, family, group); provides educational and consultation services to Providers and staff, community agencies, and the public; and performs related duties as required.

DUTIES/ESSENTIAL JOB FUNCTIONS:

  • Assess the clinical status and therapeutic needs of clients either through ongoing psychotherapy or brief consultative contacts
  • Maintain collaboration with the interdisciplinary team in recognizing, addressing and referring patients with mental illness, substance abuse and psychosocial problems efficiently and effectively to prevent further psychological or medical deterioration
  • Provide psychotherapy to underserved patients with mental illness, substance abuse and chronic diseases in an integrated community and school setting
  • Documents clinical activity presenting diagnostic and interview findings, clinical evaluations, and recommendations; and may act as a case manager by coordinating services for clients to assure continuity of care and to assure that clients receive appropriate types of service.
  • Assist in preventing relapse or further morbidity in conditions that tend to occur over time through the integrated health care modality
  • Conduct psychosocial assessments and provide brief case management
  • Formulate, implement, evaluate and review treatment plans and DSM-IV diagnoses
  • Educate patients, their support systems and staff on prevention and treatment enhancement techniques
  • Maintain an electronic medical record that accurately reflect the services provided and the patient’s growth towards treatment goals
  • Perform other duties as assigned

 

KNOWLEDGE, SKILLS, ABILITIES:

  • Proficient in DSM-IV criteria, demonstrate the ability to diagnose and apply diagnoses for treatment planning
  • Must possess strong oral and written communication skills
  • Ability to exercise good judgment and discretion in establishing work schedule
  • Must establish and maintain productive and professional relationships with patients, staff and community partners
  • Ability to work independently, effectively manage time and maintain organizational skills
  • Possess creativity in identifying problems and creating solutions
  • Knowledge and compliance of all applicable guidelines, policies, procedures and laws
  • Ability to handle confrontational situations in a professional and productive manner

 

EDUCATION AND TRAINING:

  • Master’s Degree in Social Work, Psychology or other related field
  • 3000 hours post graduate work experience
  • Current Wisconsin licensure as an LPC
  • WI Substance Abuse certification preferred

WORK SCHEDULE

Open availability to work up to 40 hours during clinic operating hours, typically Monday – Thursday, 7am – 7pm, Friday, 7am – 5pm and rotating Saturdays 7am – 3pm.
