Patient Relations Specialist Requirements & Responsibilities:

We are a reputable Home Health Care agency currently interviewing for an experienced Patient Relations Specialist for our Brooklyn Office.

Key Responsibilities:

  • Act as a liaison between patients and aides, ensuring effective communication and service delivery.
  • Address patient concerns and inquiries promptly and professionally.
  • Coordinate patient care services, ensuring all needs are met efficiently.
  • Supervise and support home health aides, providing guidance and resolving any issues that arise.
  • Manage patient records and scheduling using HHA Exchange software.
  • Maintain detailed and accurate documentation of all interactions and services provided.
  • Collaborate with the healthcare team to improve patient care services and outcomes.

Requirements:

  • At least 1 year of experience working in healthcare or home care is preferred.
  • Bilingual in English and another language is required.
  • Proficient in Microsoft Word and Excel.
  • Proficiency in HHA Exchange software for scheduling.
  • Strong multitasking and time management skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as a collaborative partner.
  • Self-driven with the ability to manage projects by establishing objectives.
  • Superior customer service skills.
  • Excellent written and verbal communication.
  • Detail-oriented with strong organizational skills.

  • English & Another Language (Spanish preferred)

Schedule:

  • 8-hour shifts
  • Monday to Friday
  • Weekends as needed
