The Care Coordinator at Serenity is responsible for guiding and assisting program residents in identifying their basic spiritual, physical, emotional, and social needs, with the goal of developing a relationship with Christ and freedom from drugs, alcohol, abuse, and homelessness.  Assists residents in developing actionable plans, and holding them accountable for working the plans.  Coordinates resources and referrals for residents until the resident is able to do so on their own.  Participates as a vital member of the ministry team and works collaboratively with other department managers and staff to ensure the best possible conditions for success are maintained.  A strong emphasis is placed on the leadership of volunteers and volunteer groups in order to fulfill the objectives of the program. 

 

Duties:

  • Develop relationships with clients that promote and model a well-balanced Christian life.
  • Demonstrated commitment to KARM’s mission and agreement with our statement of faith and guiding values
  • Establish care plans that will guide resident participation from start to finish.
  • Meet with assigned residents as necessary to ensure maximum benefit from KARM programs and services.
  • Make appropriate internal and external referrals for services.
  • May make the following resident-related decisions: authorize appointments, leisure time passes, phone call logs; needs requests, visitation requests, transportation requests; sick day requests and medication changes; chore exchanges / changes
  • Refer to or consult with VP regarding: final resident plan; changes in resident personal plan; requests for disbursement of funds from KARM; recommendations for dismissing someone from residence; recommendations for completion and graduation; recommendations for phase advancement; recommendations for consequences for serious infractions
  • Maintain accurate, up to date, file records for all assigned residents, utilizing HMIS and other applications and tools provided.
  • Act on behalf of VP as requested and/or in her absence.
  • Authorize outside appointments and leisure time passes.
  • Exhibit caring Christian lifestyle to residents, staff, volunteers, and donors; establishing a sense of trust, safety, consistency, and hope
  • Demonstrated ability to work cooperatively with peers and management toward the accomplishment of the ministry purpose
  • Serve as a member of the Serenity team, participating in team meetings and other related activities.
  • Work cooperatively with ministry staff and departments to ensure an effective and efficient residential environment.
  • Coordinate work processes with other departments as needed; work with and supervise volunteer caregivers.
  • Perform other essential duties as assigned by VP.


Requirements:

  • BS in social work, psychology, or related field
  • Residential experience as a case manager highly desirable
  • Commitment to Christian faith required; spiritually mature; Able to clearly explain and present the Christian plan of salvation and lead a person through the process
  • Two years working with the homeless and/or those with substance abuse and/or mental health challenges
  • Demonstrated skills in individual and group counseling
  • Competent computer user/user of MSOffice products
  • Good organizational and administrative skills; able to manage a busy work calendar
  • Ability to build personal relationships and maintain appropriate boundaries with residents

Working Conditions: 

  • Potential high-risk environment at KARM maintained property
  • Working daily with individuals having medical, mental health, and/or addictive behaviors
  • Some local travel may be required for various purposes
  • Occasional required attendance at KARM special events, sometimes during weekend or evening hours
