Requirements:

  • 1 Year experience required. 
  • Experience working with pregnant or post-partum women highly preferred.
  • License Required: CRPA - NYS
  • Certificate Required: CRPA - NYS
  • Bachelor’s Degree in human services, social work, or related field

OR

  • Associate’s degree with 1 year of case management experience

OR

  • High School Diploma with 2 years of case management experience
  • Excellent organizational, communication and computer skills

Position Summary

  • Accompany clients to appointment, on transportation, and assist in reducing barriers to accessing care.
  • Assist clients to identify goals, strengths, and personal interests helpful in recovery by providing didactic information.
  • Motivate clients to attend treatment programs; find ways to help clients consider stopping/reducing using substances or reducing the risk associated with addictive behaviors.
  • Utilize a trauma-informed approach to promote clients right to self-determination and provide services in a manner that emphasizes the women’s safety, respect and dignity. 
  • Participate in outreach to clients who are ambivalent about the assessment process, and discuss the process with them.
  • Facilitate recovery oriented discussions with clients in the community.
  • Become a Narcan responder and dispenser by attending training and becoming certified in dispensation of Narcan; train clients on the use of Narcan and provide kits and guidance on proper procedures of its use.
  • Promote self-advocacy and identify community resources that support clients' goals and interests and share them with clients under the supervision of M-WRAP Social Worker.
  • Performs other duties as assigned.
