Key Responsibilities- Collaborate with individuals to develop personalized recovery plans that cater to their unique needs and goals.
- Link patients to community-based social and support services, promoting a seamless transition to independent living.
- Teach coping skills and strategies to individuals and their families, empowering them to manage challenges and maintain a healthy lifestyle.
- Support the application process for benefits, ensuring individuals have access to necessary resources.
- Provide non-clinical crisis support after periods of hospitalization or incarceration, offering a safe and supportive environment for individuals to process their experiences.
- Accompany individuals to court appearances and medical appointments, providing emotional support and advocacy.
- Engage individuals with formal recovery support services, promoting a sense of community and connection.
- Educate individuals and families on various modes of recovery, empowering them to make informed decisions about their care.
- High School Graduate or Equivalency required.
- Certified Recovery Peer Advocate (CRPA) Certification required within six (6) months of hire and maintained throughout employment.
- One year of Health or Human Service experience preferred.
- An individual does not need to be in recovery themselves but must be able to confer empathy through lived experiences such as family and/or close friends.
- Must have a valid NYS Driver's License.
