GENERAL RESPONSIBILITIES:
Assist tenants functions in the work, social, living and learning environments of their choice. Service provision aimed at preventing future episodes of homelessness. Ensure that tenants on caseload receive benefits to which they are entitled.
PRIMARY TASKS AND RESPONSIBILITIES:
1. Maintain primary responsibility for an individual caseload of tenants.
2. Assessment of tenants in terms of functional abilities and service needs.
3. Development of service plans, with tenant input, aimed at assisting tenant establish and accomplish personal goals.
4. Provide necessary linkage to needed/desired services (e.g. day programs, vocational rehabilitation, legal services, medical/dental services, outpatient psychiatric service).
5. Attend case conferences focusing on coordination of treatment/services provided to tenants.
6. Assist tenants acquire skills related to daily living, socialization, budgeting, personal hygiene, maintenance of living space, shopping, traveling, meal planning/preparation and use of leisure time. Provide opportunities for socialization and program management input.
7. Provide services to assist tenants to be independent in medication management.
8. Provide crisis intervention services as needed. This may include mandatory overtime.
9. Participate in weekly staff meetings and supervisory sessions with Program Director.
10. Maintain case records and other documentation in a manner consistent with agency policies and procedures.
11. Conduct utilization reviews on cases which are not on worker’s individual caseload.
12. Inform Program Director of any problems/areas in need of attention regarding documentation and quality of care issues.
13. Attend annual anti-sexual harassment training
The above tasks and responsibilities are intended to describe the general nature and level of work performed by people in this job category. This is` not to be construed as an exhaustive list of all responsibilities, duties and skills required of personnel so classified. All personnel may be required to perform duties outside of their normal responsibilities from time to time as needed by the Program.
REQUIREMENTS:
High School Diploma (GED), Bachelors Degree preferred
Criminal Check Approval (NYS & FBI)
Experience in service provision to mentally ill adults
Bilingual preferred (English/Spanish)
Good interpersonal skills
Professional presentation
Work hours flexibility
