We are hiring for Social Service Position. 

Position Summary: 

Under the direction of the Case Manager Supervisor, provides case management services to residents residing in homeless shelters to ensure coordinated, interdisciplinary delivery of optimum quality services.  This position will ensure the delivery of services to residents using DHS policies, procedures and standards. 

Responsibilities: 

  • Assist the clients with the development of personal goals and job readiness (Individual Responsibilities Plan) which addresses potential barriers to employment and how to overcome them.
  • Conduct orientations, collect weekly timesheets, provide pre- and post-placement coaching, and update action plans are required.
  • Coordinate with other agencies to assist clients and make appropriate referrals.
  • Network with community resources to provide services to address specific barriers customers must overcome to accomplish their goals.
  • Consult with other staff to promote customer goals and career related needs.
  • Follow-up with customers who are not attending program and provide intensive support to ensure compliance.
  • Monitor and record clients' progress to ensure that goals and objectives are met.
  • Prepare and maintain hard copy case files, including documentation such as clients' personal and eligibility information, services provided, client contacts, and relevant correspondences.

Requirements:

  • College degree preferred in related field or equivalent work experience.
  • 3-5 years’ experience as a Case Manager or working with out of school youth ages 18-24.
  • Must be able to work independently or in a team environment.
  • Ability to assess individual skills with precision and make appropriate referrals.
  • Ability to problem solve and work with a diverse customer base..
  • Must have the ability to work well with all levels of the organization and individuals with multiple barriers..
  • Must be well organized, detailed oriented, and accuracy driven.
  • Must be able to handle assignments quickly and efficiently, meet deadlines, and handle confidential matters and mature and responsible manner.
  • Must exhibit excellent interpersonal, organizational, and written communication skills.
  • Experience supporting at-risk youth entering into employment and/or post-secondary education preferred.
  • Must be proficient in Microsoft Office software including Word, PowerPoint, Excel, and Outlook.
  • Prior Case Manager Supervisory a plus.
