ABOUT THE MEDICAL ASSISTANT (MA) / HEALTH AND WELLNESS COORDINATOR (HWC)
Upperline Health is seeking an experienced Medical Assistant to support a team of clinicians in our Lake Nona clinic delivering complex plans of care in accordance with the patient’s health status and overall goals and values. In addition to clinical support, this individual will be providing care coordination, including all telephonic and back-office support required by the team of clinicians.
In this role, you act as a patient’s guide to a complex and confusing health care system. You will provide critical support activities to coordinate personalized care delivery for Upperline patients and patients’ families.
Ideal candidates will have experience identifying and coordinating the needs of patients with multiple chronic conditions and supporting clinicians to help them operate as efficiently as possible. Experience with an older adult / geriatric population is strongly preferred. Successful candidates should be excited and energized by the prospect of working collaboratively as part of an innovative, patient-focused care team. WHAT YOU’LL DO
  • Place patients in exam rooms, ensuring that they are ready and comfortable for their appointment with the physician
  • Conduct health assessments, complete exam intake, including but not limited to, reviewing the patient's medical history, social history, surgical history, and the history of present illness and complete visits with patients being seen in our Podiatry Clinic.
  • Complete vitals and required documentation in clinic EMR (Athena Health).
  • Care coordination activities assigned by an Upperline Plus Nurse Practitioner, including but not limited to, medical records requests, order management, appointment scheduling, managing patient care referrals, etc.
  • Provide patient care under the direction of a podiatric physician including taking and developing x-rays and assisting in the exam room as required
  • Perform daily administrative tasks such as stocking all exam rooms and disposing of exam room trash
  • Responsible for cleaning and sterilizing exam and x-ray rooms both after each patient visit and at the end of each day and sterilize instruments daily
  • Satisfy and document appropriately required MIPS quality measures
  • Facilitate and support virtual provider encounters in the clinic, telephonic outreach and engagement with Upperline Plus patients to coordinate ongoing preventative care.
  • Participation in weekly interdisciplinary team meetings.
  • Responsible and accountable for Upperline Plus panel management
  • Contribute to daily huddle facilitation & leadership
  • Provide routine foot care for at-risk patients with Diabetes and/or other chronic illness
  • Telephonic outreach and engagement with Upperline Plus patients to coordinate ongoing preventative care
  • Identifying community resources and assisting Upperline’s patients and families in accessing those resources

QUALIFICATIONS OF THE MA/HWC
  • A minimum of 2 years’ experience as a medical assistant providing direct patient care is required.
  • Experience in an outpatient clinic setting is strongly preferred.
  • Clinical care coordination or care management experience is ideal.
  • Previous experience assisting or providing care to the adult – geriatric patient population is preferred.
  • Prior wound care and foot care experience is a plus.
  • X-ray experience is a plus
  • Bilingual English/Spanish is helpful.
  • Previous experience working in an EMR system – ideally, AthenaHealth.
  • Database management skills and proficiency using Word, Excel and other Microsoft Office products is necessary.
  • Must have reliable transportation, valid driver’s license and minimum state required liability auto insurance.
  • Extremely strong customer-service orientation and work ethic.
  • Exceptional organizational skills and ability to multi-task.
  • Self-starter with an entrepreneurial spirit and demonstrated problem solving skills.
  • Excellent oral and written communication skills to ensure that each of your interactions with patients, family caregivers, clinicians, and team members is effective and meaningful.

Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience
