LifePath is a mission-driven, collaborative, and community-focused human services agency seeking a full-time (35 hr/wk) Resource Specialist.

The Resource Specialist is a member of the Information and Caregiver Resource Center (ICRC) team. ICRC provides information and referral, follow-up and advocacy to callers and processes intake information for internal programs operated by LifePath. The Resource Specialist serves as the public’s entry point to LifePath’s services and also has responsibilities in LIfePath's Expanded Information and Referral; Massachusetts Family Caregiver Support Program; and Options Counseling.

This is a full time position (M-F, 35 hrs per week); pay is $21.60 per hour, generous benefits.

QUALIFICATIONS:

Bachelor’s Degree in a Human Service related field preferred with years’ experience working with elders and caregivers

The position requires:

  • Knowledge of community resources and ability to research options
  • Sensitivity to work with diverse populations in their homes, virtually and telephonically.
  • Comfort with individual and group outreach events and presentations.

  • Ability to travel throughout Franklin County and North Quabbin region

ESSENTIAL FUNCTIONS / DUTIES & RESPONSIBILITIES:

  1. Conduct consumer interviews to determine individual needs and options.
  2. Complete intake assessments based programs including MassHealth screenings for internal agency programs
  3. Assist consumer in developing a plan of care that is consumer-directed and includes services, programs, financial resources and other supports identified by the RS or consumer.
  4. Maintain up-to-date and comprehensive information of available resources for elders, caregivers and individuals with disabilities.
  5. Maintain current understanding of relevant regulations and application procedures of entitlement benefits.
  6. Conduct community presentations and outreach activities through public speaking engagements or related activities.
  7. Provide follow-up communication with consumers as needed to ensure outcomes of previous contact and offer additional assistance in locating other services as necessary.
  8. Maintain accurate and up-to-date documentation according to program and agency requirements. of Information and Referral (I&R) calls, follow-up and advocacy work. Complete daily call log in database on all calls.
  9. Participate in activities to assist the Aging and Disability Resource Consortium (ADRC) in gaining visibility and continued collaboration between LifePath and community agencies, providers, etc.
  10. Participate in regular agency meetings and team meetings and attend in-service trainings.
  11. Maintain accurate and up-to-date documentation according to program and agency requirements. of Information and Referral (I&R) calls, follow-up and advocacy work. Complete daily call log in database on all calls.

Physical and Mental Demands:

Physical: Visual, speaking, auditory and mobile capacity necessary:

Environmental and Working Conditions

Work is split between indoor office work and traveling outdoors to meetings, trainings and consumers’ homes.

Must be able to tolerate odors and pollutants including but not limited to smoke, air fresheners, pet dander, personal odors, etc. from a variety of in-home conditions.

Must be able to tolerate by-products of office machine operation.

Must be able to tolerate heat and cold of seasonal changes and indoor temperatures.
