Homeless Outreach Nurse

Wanted:  Versatile individual to join our team at Hands On Hartford as Homeless Outreach Nurse with Hands On Hartford (HOH).  The Homeless Outreach Nurse, will work with individuals experiencing homelessness in the Greater Hartford Area to support optimal health and well-being.  This position will promote the well-being of the community and improve the quality of life among people experiencing homelessness. As a priority, the role will engage in activities that will promote, maintain, and improve the health of unhoused individuals, while advocating and providing social support and informal counseling for individuals experiencing homelessness   and community health needs.  This position will work collaboratively with our outreach team, local municipalities, community agency partners, and contract providers to ensure that they health needs of unhoused individuals are met.

When:  Full time; schedule consists of early mornings, evenings and weekends.

 

Pay:  Range $33-36 per hour.  This is an hourly position.

 

As the Homeless Outreach Nurse: At least 1 year of nursing experience; experience with street outreach or other Emergency Services working with unhoused individuals, preferred. Excellent communication skills, particularly listening, mediation, and writing skills; possess strong organization skills with the ability to meet a demanding workload; knowledge of and commitment to services related to working with individuals experiencing homelessness, HIV/AIDS, addiction and mental health; knowledge of Housing First and Harm Reduction philosophy.   Fluency in Spanish and English preferred

Qualifications: 

·         Ability to provide services in a culturally competent manner and ensure trauma sensitive care.

·         Fluency in English and Spanish is a plus.

·         The ideal candidate will have proficiency in Microsoft Word, Excel, Outlook and other Microsoft products.

·         Strong written and verbal communication skills.

·         Valid Driver’s license with insured vehicle required.

·         Ability to lift and carry up to 25 lbs.
