Case Management Supervisor

Job Title: Case Management Supervisor
Location: Detroit, MI 48202
Duration: 6 months of a contract
Schedule: 8a-4:30p M-F
Job Description:
1. Responsible for daily operations of the case management department. This includes the supervision of the delivery of initial psychosocial assessments (opening cases), and follow-up assessments performed by the case managers for discharge planning.
2. Maintains current knowledge of resources available within the community; communicates and maintains the supply of resources materials to be distributed to hospital case managers.
3. Ensures the accurate and timely implementation and evaluation of new and existing programs to achieve departmental and organizational goals and objectives.
4. Engages and evaluates competency-based performance of staff members on a routine and as needed basis; develops educational curriculums and tools; provides immediate feedback to staff members regarding performance.
5. Partners with key stakeholders along the continuum of care to provide daily review for complex or catastrophic cases and treatment plans, communicates with the attending, consulting and/or ED physicians.
MUST HAVE Epic and Careport experience
MUST HAVE current acute care case management leadership experience (2 year minimum required)
MUST HAVE inpatient hospital case management experience
MUST HAVE discharge planning and utilization management experience
MUST HAVE case management experience, no time to train on the basics
MUST HAVE RN or LCSW license Master's degree required for LCSW but BSN required for RN
