Chester-Lancaster Disabilities and Special Needs Board, Inc. - Case Manager

Employment: All agency staff are employed to serve people with disabilities and special needs, whether in a direct support position, supervisory role or management position. A Case Manager is the entry point for all new referrals and is responsible for the overall implementation of coordinating the processing of new referrals into the DSN service delivery system.Required qualifications for the position:Bachelor’s degree in Social Work or related field (e.g. Sociology, Psychology, and Special Education) from an accredited college/university or a Bachelor’s degree in an unrelated field and at least one year experience in programs for persons with disabilities or have at least one year of experience in a case management program and be able to demonstrate knowledge of disabilities.

Duties and Responsibilities: Acts as an entry point for all new referrals,Coordinates the processing of new referrals into the DSN delivery system. Secures initial social history during home visit, Schedules the diagnosis and evaluation of consumer with the DSN program,Conducts an interpretive interview with the consumer and family/guardian regarding findings and recommendations of the evaluation, Coordinates access to all services available to persons with mental retardation, including those available to Medicaid recipients and other necessary services within a community.Develops initial Plan of Service for new referrals entering the DSN service delivery system. The Plan of Service is further developed by the Case Manager after eligibility determination from the consumer’s assessment Team and after thorough consultation with the consumer requesting services, family members, physicians or nurses, or other professional services, involved with the individual. Reviews the Plan of Service with the consumer and his/her family. It is the responsibility of the Case Manager to update the Plan of Service as needed. Completes an Annual Needs Assessment/Single Plan on an annual basis, a quarterly review of the Needs Assessment and update as needed. Request input from the consumer and his/her family regarding service needs and documents this on the Individual Needs Assessment/Single Plan. Assists consumers in obtaining needed services identified in the Plan of Service including all services covered by Medicaid, as well as, those not covered by Medicaid.Coordinates services from multiple agencies that are required to meet the consumer’s needs. Attends public school meetings, community support meetings, and meetings with any other organization on behalf of the consumer.Serves as the consumer’s advocate in assuring access to a wide range of services. At all times the Case Manager will assure that the consumer’s freedom of choice of providers is maintained in accessing services (Medicaid and Non-Medicaid, including choice of service coordinator). Will coordinate access to primary care physicians, local DSS programs, county health departments, and other local service providers. Coordinates services within local DSN program(s) and affects transfers to appropriate services within the DSN program(s) that are indicated by the consumer’s Plan of Service. Freedom of choice will be maintained at all times, including freedom of a consumer to receive DSN services from a program other than that which employs the Case Manager, if available, including choice of Case Manager. Provides on-going supervision of Plan of Service and documents required activity on each consumer serviced by the Department of Disabilities and Special Needs. The receipt of service coordination services must be at the option of the consumer and/or the family/guardian. The consumer, family member and guardian must also be free to choose the frequency of the case management contacts. The frequency of activity will be based on the consumer’s needs, but at a minimum, quarterly.
