Position Summary:

The Outreach Specialist under the general supervision of the YEPP Program Coordinator will work collaboratively with the YEPP team to assist with developing, facilitating and promoting community events to help prevent the use and addiction to drug and alcohol substances by youth ages 12-17.

The Outreach Specialist will also collaborate with managers and care providers from multiple disciplines/departments to manage and improve health outcomes and the patient experience of care for Watts Healthcare Corporation’s (WHCC) at-risk patient populations, as defined by each patient’s risk profile and other social determinants of health.

Essential Functions and Responsibilities:

  • Assist in developing, organizing and maintaining community engagement strategies and outreach focusing on youth prevention of substance use and abuse.
  • Facilitate prevention community seminars, educational sessions and workshops.
  • Establish relationships with internal and external stakeholders to educate and promote a safe and healthy environment for youth.
  • Act as liaison between community and external agencies distributing informational materials to help support the goals of the YEPP program.
  • Attend monthly evening community meetings.
  • Attend to phone calls and messages as it applies to YEPP program needs.
  • Have excellent verbal and written communication skills in order to effectively relay information to others.
  • Maintain communication with other local service agencies for youth referrals as needed
  • Have an energetic disposition, enthusiastic and able to handle multiple tasks with a positive attitude.
  • Have a good disposition when working with others.
  • Develops network of community resources and maintain liaisons with these resources
  • Be available to translate for specific sessions, if qualified.
  • Must have a current/valid California Driver’s License and proof of insurance.
  • May be assigned to work at a satellite clinic/site on a scheduled, temporary basis as needed.

Other Experience and Skills Required:

  • Ability to work effectively in an interdisciplinary team approach with both paraprofessionals and professionals.
  • Ability to maintain client dignity and confidentiality.
  • Willingness to work non-regular hours may be required.
  • Bilingual (Spanish/English) is strongly preferred.
  • Familiarity with community resources desirable.
  • Excellent writing and verbal skills are essential.
  • May perform other job-related duties as required.

QUALIFICATION REQUIRMENTS:

Education and/or Experience:

  • High school diploma or equivalent.

AND

  • One-year experience working with youth.
