INTAKE REPRESENTATIVE

Under the supervision of the Intake Manager, the PACE Southeast Michigan Intake Representative is responsible for coordination of the PACE intake process including performing initial screening, informing potential participants of eligibility requirements, maintaining the Home Visit schedule, and reporting intake activity data collection. Serves as the initial liaison between potential participants and their families, and the PACE team. The intake representative will deliver information about the PACE program and other community services through phone contact to promote enrollment into the PACE program.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

• Responsible for all aspects of intake process including responding to inquiry calls, , providing information, conducting initial phone screening, and assisting Outreach Liaisons with scheduling home visits for referrals.

• Documents intake activity through completion of phone screen, documentation in referral module in TruChart, and communication with the Intake team.

• Completes quarterly referral and activity reports. Responsible for participant information input into various databases.

• Maintains the Enrollment module in TruChart.

• Communicates with Home Visit staff for daily home visit assignments.

• Coordinates timely follow-up with referral sources, families, and participants through phone contact, letters, and all other appropriate documentation.

• Assists in community outreach activities as assigned. Responsible for PACE representation.

• Keeps confidential, all PACE or prospective PACE participant information, and/or all other information pertaining to PACE or prospective PACE participants, issues or business practices.

• Other tasks as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:

• High School diploma or GED required.

• One or more years’ experience working with elderly in a community-based setting required.

• Must have knowledge of senior community and area resources.

• Have knowledge, skills and competencies to work effectively with populations of diverse background and a variety of ages, including one year of experience working with the elderly.

• Requires the ability to function in a multi-task environment.

MUST POSSESS THE FOLLOWING QUALITIES:

• Possess interpersonal skills and communication skills

• Be open to new learning experiences

• Demonstrate customer service values

• Requires basic knowledge of Microsoft Windows programs such as Word and Excel.

• Must be medically cleared for communicable diseases and have all immunizations up-to-date after engaging in direct participant contact.

• Completes all annual, mandatory in-service training and screening, including but not limited to:

  1. Infection Control
  2. TB Testing
  3. Emergency Preparedness
  4. HIPAA
  5. Ergonomics and Participant’s Rights
