Supported Living Specialist

Hours: Primarily Monday through Friday, eight hour shifts with flexibility as needed
Position Summary:
The Supported Living Specialist helps participants manage their daily living activities both at home and in the community. They assist with overcoming challenges, achieving goals, and offer support with health and wellness, budgeting, employment, and maintaining an independent lifestyle.
Education and Experience:
- Minimum of two years of direct experience in supporting people with Intellectual and Developmental Disabilities (IDD).
- This position prefers a bachelor’s degree from an accredited college or university.
Required Skills/Abilities:
- Proven capability to effectively manage a wide range of responsibilities.
- Take initiative with problem solving and navigating services.
- Proficiency in budget management and financial oversight.
- Proficiency in Microsoft Word and Excel.
- Valid driver’s license with insurability and a clean driving record required.
- Candidates must have the ability to pass a qualifying criminal background check and drug test.
Physical Requirements:
- Must be able to lift 15 pounds at times.
- Must be able to access and navigate throughout the community, participant’s homes and the organization's facilities.
Duties/Responsibilities:
- Providing supports in alignment with the National Alliance of Direct Support Professionals Code of Ethics and Competencies.
- Develop trusting and meaningful relationships with participants and the teams that you work with.
- Promptly address phone calls, emails, and other communications from participants as well as the team including Internal Support Coordinator (ISC) and family members, if appropriate.
- If applicable to the person, support the person in being more independent with personal hygiene. Providing personal support requires a sensitive and respectful approach, ensuring the dignity and privacy of the person are always respected.
- Provide guidance and support to participants in various work and home activities by doing with and not for them. This includes but is not limited to shopping, carrying in groceries, cleaning, vacuuming, sweeping, and completing laundry to maximize their independence.
- Ensure precise completion, maintenance, and timely submission of all necessary documentation and written records.
- Follow all support plans designed to address the participant’s independence and overall quality of life.
- Support participants with utilizing community services, overcoming crisis, problem solving and evaluate needs.
- Contribute to a culture of safety by following established health and safety procedures. Following the rules and regulations to include Department of Health, Council on Quality and Leadership, Administrative Rules of South Dakota, Department of Developmental Disabilities, Health Insurance Portability and Accountability Act, Home and Community Based Services Settings Rules, etc.
- Drive agency vehicles to support people to work and various community activities.
- Accountable to manage and report on individual finances in accordance with agency policy and industry best practices.
- Assist individuals with budgeting, financial monitoring, and training as requested.
- Continuously monitor progress and inform the Internal Support Coordinator (ISC) of any changes in the person's goals, support requirements or service authorizations.
- Meet with the person to review their plan and assess their satisfaction with the services provided.
- Follow due process procedures in handling potential rights restrictions, grievances, suspected cases of abuse or neglect, and legal issues.
- Support people by recognizing them as adults, coaching rights with responsibilities and support people with the freedom to take risks and make choices that enhance their quality of life, even if those choices involve some level of risk.
- Responsible for developing and nurturing community connections, assist people in creating personalized schedules, and enable their participation in activities of their choice.
