The Case Manager role consists of being an integral part of the treatment team for patients by developing an aftercare and discharge plan, completion of FMLA, return to work letters, and legal matters that patients may be facing. You will be a key player in the treatment team for each patient and will also meet with them periodically to assess their case management needs and their goals for when they leave our care.
Job Duties:
- Assessing patient needs and developing strategies to help the patient while in treatment
- Collect patient signatures on needed documentation
- Assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process
- Liaising with government entities and personnel as needed to assist in the handling of legal matters
- Work with the patient, family, and friend support networks and other members of the care team to craft the discharge and aftercare plan
- Providing local resources for patients for aftercare outside of the continuum of care
- Proper record keeping of patient meetings, referrals, and updates to care or legal matters
- Evaluating clients progress periodically and making adjustments as needed to improve outcomes
- Following up with discharged clients to ensure they were satisfied with services and they are still in good mental health and abstinent from substances
- Any other duties or tasks requested by the supervisor or upper management
Requirements and Qualifications:
- Minimum of one year of experience in the behavioral health care field, specifically within case management
- Knowledge of State & Federal Statutes Regarding Patient Confidentiality laws
- Knowledge of Drug-Free Workplace Policies
- Knowledge of Workplace Violence
- Knowledge of Corporate Integrity & Ethics
- Ability to work within a team structure and have strong interpersonal and communication skills
- Knowledge of Completion of FMLA, Short Term Disability, and Social Security
- Knowledge of Joint Commission, DCF/DHCS, and AHCA
- Working knowledge of duty-related software and Microsoft360
