Provide guidance and support to people with intellectual and developmental disabilities.

about health and safety, relationships, and rights.

Minimum Qualifications:

High School Diploma/GED with at least 4 yrs. Experience working with people with intellectual and developmental disabilities; or

AA degree in a related field with at with at least 2 yrs. Experience working with people with intellectual and developmental disabilities; or

Bachelor’s degree in a related field. Must be able to pass the State course for Supported Living, Supported Employment and Social Security Incentives.   At least 18 years of age, dependable transportations, valid Florida driver’s license and proof of insurance and registration.

 Hours cannot be determined as they will change according to customers’ needs.  However, typically SLC hours of operation are Monday through Friday between the hours of 8:30am and 7:30pm. SLC’s are also expected to maintain a weekly rotating schedule with the team of coaches and management for the 24-hour on call services. In addition, SLC’s are expected to be always on call to the agency emergency phone for potential questions or assistance needed. 

  • Complete customer assessments to define level of training and education needed in all areas of daily life including relationships, medical and health concerns, personal care, household and money management, community mobility, recreation and leisure. Assist customer with developing an implementation plan according to their choices and needs.
  • Track and ensure payment of customer bills (Teco, phone, Utility bills, Rent).
  • Reviewing all customer medication to ensure proper documentation and administration of mediations, during onsite visits.
  • Maintain all customer files, as result of file being monitored.
  • Any program monitoring preparation.
  • Provide training and support according to the customer’s support plan goals.
  • Assist customer with behavior management using specific techniques/procedures based on the customers behavior plan, as needed to maintain the safety of all individuals present.
  • Training, educating, reviewing and critiquing all required documentation for all participants of assigned program, during on site visits.
  • Random visits with the customer, while the employee is working to ensure that schedules are being followed and that quality services are being rendered.
  • Provide transportation for customer in a safe responsible manner at all times, including abiding by all motor vehicle laws for the state of Florida. Conduct all responsibilities in a professional, responsible manner at all times.
  • Inspect customers home to ensure all safety needs are met and maintained, including staff living area. (Detectors, carpets, utilities, walls, odors, entrance way, etc.)
  • Ensure all goals according to the customers' support plan and implementation plan are being executed by all staff.
  • Ensure that all customers are being treated with respect and dignity by all staff personal.
  • Ensure that all staff personal are following doctor’s orders (diet, exercise, medication, etc.)
  • Conduct and maintain responsible, professional communication with all members of the community, family members, support coordinators and all others involved in the customer’s daily life.
  • Attend all required trainings (approximately 18 hrs. per year), staff meetings, quarterly and support plan meetings upon request.
  • Provide feedback and consistent communication with Operation Manager and Executive Director regarding issues as needed.
