Job description
We are a Federally Qualified Health Center located in West Philadelphia. We are looking for a dedicated and culturally competent Behavioral Health Consultant to join our team. Our busy health center is focused on providing care to an underserved urban population. We do provide our patients with extended hours, two days during the work. This position is responsible for working extended hours from the date of hire. The hours are listed below for your review. You will join a team of dedicated providers providing comprehensive primary care services. All clinical notes must be completed within 24 hours of seeing your patients. Our health center mission is rooted in education as well. Our site has medical students rotating on site; thus, you have the opportunity to teach as well.
Job Summary
Provide billable mental health services / counseling to our patients. Enhance the impact of the health center in the community and by community members, especially patients, with mental health and substance use disorders by the provision of social health services, in support of our mission to provide accessible and culturally sensitive high-quality community-based primary and preventive health care services to the underserved West Philadelphia community and offer trauma informed psychoeducation to students from high school to the post-graduate level.
Essential Functions
1. Provide billable substance misuse services to current patients. Screen and assess patients for substance use disorders, e.g., AUDIT, CAGE AID. Prepare documentation in the electronic medical record.
2. Provide billable mental health services to our current patients. Screen and assess patients for mental health disorders using standard screening tools, e.g., PHQ-9, GAD-7, PCL-5. Prepare documentation in the electronic medical record.
3. Deliver cognitive and behavioral interventions using evidence-based treatment approaches, e.g., Screening, Brief intervention, referral to treatment (SBIRT), motivational interviewing, problem-solving therapy (PST), acceptance and commitment therapy (ACT), etc.
4. Assist patients in obtaining social and vocational services, as needed, e.g., medical assistance, housing and utility assistance, vocational rehabilitation. Track patient follow-up and outcomes using electronic medical record.
5. Cultivate and maintain collaborative relationships with local behavioral health organizations and providers. Refer patients to specialty behavioral health providers as needed. Track patient follow-up and clinical outcomes using the electronic medical record.
6. Provide education to patients, staff, medical providers, and community constituencies on a wide spectrum of topics related to integrated behavioral health, mental health and substance use disorders, with an emphasis on reducing stigma associated with mental health and substance use diagnoses.
7.Coordinate with the Outreach team for outreach outings in the Philadelphia and surrounding area
8. Perform additional duties as assigned by the Medical Director and/or the CEO.
Scope/Challenges
Provision of billable substance abuse and behavioral health services requires coordination, collaboration, and outreach.
Must maintain compliance with grant expectations or grant-funded programs.

Required Education and Experience
LCSW or Psychiatric Mental Health Nurse Practitioner (PMHNP)
At least 2 years of social work and clinical experience
Eligible to participate in federally funded health care programs including Medicare and Medicaid as well as other managed care and fee for service products.
Training in addiction counseling

Salary: $90,000 per year based on experience.
Schedule:
8-hour shift starting at 11:00 a.m. to 8 p.m. (Tuesday and Thursday). 8 a.m. to 5 p.m. shift (Monday, Wednesday and Friday). Some weekends with events with our Outreach Department.
