Your Responsibilities

The Care Coordinator is a role that provides support in the office reporting to the Care Coordinator Manager and supports in the field as needed.

  • Treat Clients and fellow Team Members with the highest level of respect and compassion
  • Schedule and coordinate day to day activities of our Caregiver Team Members
  • General office administrative duties
  • Provide pertinent documentation to the Care Coordinator Manager on employee performance
  • Report pertinent client and employee information during weekly staff meetings.
  • Responds promptly and courteously to all clients’ and caregiver calls.
  • Communicates continually with employee and client to evaluate service.
  • Provide backup Caregiver coverage for potential open shifts
  • Works "On Call" for after-hours scheduling substitutions on a rotating basis with co-workers
  • Report any unusual incidents to our Care Coordinator Manager
  • Act quickly and responsibly to emergencies
  • Participate in the after hours on-call/Weekend Runner structure on a bi-weekly basis (frequency subject to change)

Working Location

The Care Coordinator will be based in the Portage Office location. Fresh Perspective Home Care provides quality care in Kalamazoo, Van Buren, Allegan, Cass, Berrien, and St. Joseph

Requirements:
  • A core belief that all individuals deserve respect, compassion, and caring support
  • Ability to communicate effectively with Clients, their families, and Fresh Perspective Home Care Team Members
  • Computer Skills including email and Microsoft Office Suites.
  • Prior experience with Clear Care or similar scheduling software preferred but not required
  • Prior Caregiver experience is preferred but not required
  • GED or High School diploma
  • Current and valid driver’s license
  • Fully insured reliable personal transportation
  • Ability to lift - up to 40 lbs and perform the duties of the position
  • Exhibit mature responsible behavior and uphold the patient confidentiality requirement.
