Classification: Full-time (40 Hours), Exempt Reports to: Adult Services Coordinator

Work Location: 50% Remote/50% Community or In-Person

This is a remote-friendly position, with the requirement to report in-person for standard job duties, mandatory trainings, staff meetings, and other events at the request of the Adult Services Coordinator or Executive Director

The Supportive Housing Coordinator is a behavioral health professional who is experienced in providing case management services to adults in various settings. The coordinator will support the culture of a community-based, client-centered, and trauma-responsive organization. The position will focus on supporting individuals as they transition into permanent supportive housing. This is a statewide program, and the coordinator is expected to provide services throughout the state of Maryland.

This position requires a valid driver’s license and the ability to operate a personal vehicle as a condition of employment.


Major Duties & Responsibilities:

• Works in partnership with the Local Behavioral Health Authorities/Core Service Agencies and Residential Rehabilitation Programs (RRP) throughout the state of Maryland to identify and assist individuals in transitioning from a RRP general level of care to permanent supportive housing (PSH)

• Ascertains the housing needs and available resources of individuals transitioning into PSH and works with them to determine what is available to them

• Acts as a liaison with landlords, assisting individuals throughout the transition process, and conducts regular check-ins once individuals are housed

• Ensures housing units in their jurisdictions are inspected prior to move in and meet the minimum HUD housing quality standards (HQS)

• Successfully completes HQS training within 60 days from hire and maintains certification

• Ensures the housing units are leased at Fair Market Housing rates and the participants’ incomes are 50% below the median income limits for the jurisdiction

• Ensures participants receiving rental assistance contribute at least 30% of their income towards the rent and utilities

• Participates in monthly meetings with the Behavioral Health Administration (BHA) to discuss the progress of the initiative

• Completes quarterly reports as outlined by supervisor

• Acts as a representative of the Core Service Agency to the Maryland Department of Health, BHA and Harford County Government

• Develops and maintains a good working relationship with consumers, providers, advocacy organizations, State and County officials and all other stakeholders

• Performs all work in a culturally responsive manner consistent with the agency’s mission and philosophy

• Performs other duties as assigned


Education/Experience:

Minimum requirements for this position include a bachelor’s degree in Social Work, Psychology, or another social service-related field. An incumbent must have three years of experience providing case management services. Preference will be given to candidates with strong housing expertise and knowledge of mental health and/or substance use disorders.


Physical Requirements:

• Prolonged periods of sitting at a desk and working on a computer.

• Must be able to lift to up to 10 pounds at times.

• Must be able to access and navigate community settings as necessary.
