Job DescriptionJob Description

The CCM Care Advocate is a licensed healthcare professional (ex: MA, or LPN) who supports the development of patient-centered, team-based care. S/he will support primary care physicians (PCPs) and practices in managing their panel of patients using a population management informatics tool. By gathering and organizing patient data, the CCM Care Advocate works to identify patients’ unmet needs, engage patients in their own care, gather summary information for treatment interventions, and enhance ongoing communication between the patient and her/his care team. The goal of the Chronic Care Management program is to facilitate high-value, patient-centered care that improves timely access to and provision of preventive services and chronic disease treatment.

  1. Develops a keen understanding of primary care practice requirements for optimal, coordinated population health
  2. Works as an effective team member of the care team
  3. Facilitates a Connected Care Management platform to support patients with multiple chronic diseases and assists in the coordination of the patient’s care.

  1. Manage patient registries and provide the members of health care teams in designated practices with the data required to meet the health needs of the patient
  2. Contributes to a positive experience for patients and families through courteous telephone and digital interactions, accurate and expeditious routing, as well as referral to appropriate clinical staff when necessary
  3. Contributes to the teamwork within and between departments. Regularly attends and participates in meetings with coworkers and practice staff.
  4. Perform all job functions in compliance with applicable federal, state, local and company policies and procedures

  • Minimum of 1-year experience in a direct patient care setting or similar specialty.
  • Proven problem-solver with the ability to multi-task
  • Prior use of electronic health records and other health care information systems desirable
  • Education of High school or equivalent (Preferred)
  • Clinical Healthcare Experience: 2 years (Required)
  • Certified Medical Assistant, or LPN, (Required)
  • Telehealth or remote working environment experience (Preferred)

  • Work is sedentary
  • This is a Hybrid role and work requires occasional travel to our Brunswick GA location,
  • Work requires long periods on the telephone and/or computer communicating with program participants, providers, colleagues, and other internal and external stakeholders
  • Work requires finger dexterity adequate to use computer keyboard, operate telephone
  • Work requires hearing and/or visual acuity adequate to support successful communication
  • Work requires ability to maintain calm and professional demeanor during situations that involve health of program participants and may be time sensitive and/or involve multiple interruptions; ability to perform multiple tasks without sacrificing detail orientation
  • Job Type: Full-time and Part-Time available
  • Work Location: Remote
  • Salary: $20.00 per hour
  • Position Schedule: Monday-Friday; 8:00am-4:30pm CST (can be flexible if wanting to work later in the afternoon/early evening)
