Job DescriptionJob Description

Job Summary:

The Discharge Planner develops and implements all aspects of the discharge plan for each patient, demonstrates knowledge of regulations influencing discharge plans, and post-hospital care and participates in data collection efforts for quality assurance and quality improvement purposes.

Essential Functions and Responsibilities:

  • Able to assess and reassess the discharge needs of the patient.
  • Must be able to adapt quickly, work independently, detail oriented and well organized.
  • Works with the patient and the multidisciplinary treatment team to develop a clinically and patient focused discharge plan.
  • Maintains regular, timely and appropriate documentation in patient charts regarding services provided including actual and attempted regarding appointment day/time, transportation, and any specifics related to problems associated with implementing the discharge plan and actions being taken to solve problem(s) including the overall effectiveness of the action plan.
  • Assists patient and family in addressing financial and other problems that affect discharge plan.
  • Reviews and discusses discharge plan with patient and/or family receiving input from them as well as providing explanation, information and support for acceptance and implementation of the discharge plan.
  • Functions as a treatment team member, keeping the treatment team members apprised of tentative discharge date and works with the patient and family to facilitate appropriate discharge from the hospital.
  • In conjunction with the treatment team, appropriately addresses and/or resolves, if possible, problems that delay or interferes with discharge from the hospital.
  • Shows good knowledge of referral/placement process and available community resources.
  • Performs all other duties as assigned.
  • Supports and demonstrates the organization mission and philosophy through self-examples.
  • Encourage therapeutic milieu.

Minimum Qualifications:

Education

  • Bachelor’s Degree in Social Services required, Master’s degree in Counseling or Social Work, preferred.

Experience

  • 2 years Hospital experience preferred.
  • 2 years Experience working in an acute care psychiatric setting strongly preferred.
  • 1-3 years of working experience in evaluation and treatment of mental illness.

Certifications, Licenses, and Registrations

  • BLS obtained during orientation period and maintained.
  • De-Escalation and restraint training obtained during orientation period and maintained.

Skills and Abilities

  • Ability to perform in a positive, courteous, flexible and adaptive manner.
  • Able to exercise independent judgment and initiative.
  • Experience using an electronic health record is a plus.
  • Bilingual English/Spanish is preferred but not required.

Physical Demands:

  • Must be able to wear personal protective equipment (PPE) (mask, gown, gloves) when required.
  • Sit or stand for a prolonged period of time.
  • Regular working with and viewing of computer screens.
  • Ability to lift up to, but not limited to 25 pounds.
  • Requires bending, stooping, and walking.
  • Requires good body mechanics to prevent injury.
  • Participates in de-escalation.

This position is classified as non-exempt under the Fair Labor Standards Act (FLSA). Employees in non-exempt positions are entitled to receive overtime pay for any hours worked over 40 in a workweek in accordance with state and federal regulations.
