Job Summary:

The Case Manager for La Red Health Center identifies homeless targeted populations and provides a variety of service to those in her area. He/she works with area businesses, county food banks and shelters and schools to provide safety and assistance. In addition, the medical and behavioral health needs of the community are referred to La Red Health Center. The case Manager develops strategies for outreach activities with the homeless and works collaboratively with other Outreach Teams.

Essential Responsibilities:

Incumbent(s) may not be required to perform all duties listed and may be required to perform additional, position-specific duties.

  1. Conducts intake interviews to assess the needs of the client.
  2. Once needs have been determined, plans a course of action coordinating with variety ofSussex County outreach providers for food, clothing and shelter to meet the client’s needs.
  3. Obtain medical and employment history and sets up medical appointments and mental health or family counseling as required.
  4. Sets and observes appropriate boundaries with clients, observe client confidentiality and HIPAA protocols.
  5. Ensures resources are used by eligible households who are homeless or at the risk of homeless.
  6. Maintains and documents regular contact with clients: health center visits, telephone contact, home visits.
  7. Provides transportation, as required, to health center, or other required service agencies.
  8. Attends appropriate local, regional and national meetings on homeless issues to seek out new current information.
  9. Participates in team efforts to make La Red a welcoming environment for clients utilizing our services including persons participating in our homeless case management.

Education and/or Experience:

Bachelor’s Degree in relevant field or

2 year previous experience working with multi-ethnic populations required.

Prior experience engaging in outreach efforts in rural areas is preferred.

Language Skills:

Bilingual skills preferred

Performance Requirements:

Knowledge:

  • The position requires Haitian and Latino cultural competence
  • Knowledge of the local communities and specific locations to reach the targeted population
  • Knowledge of available resources.
  • Report writing ability
  • Computer proficiency with Microsoft office series and Outlook.
  • Experience in conducting trainings and presentations.

Skills:

  • Excellent interpersonal skills
  • Excellent organizational skills and ability to self motivate
  • Experience in case management

Ability:

  • Flexibility and ability to adapt to changes and busy work environment.
  • Ability to work under pressure, address multiple priorities and meet deadlines.
  • Ability to identify key issues from interviews.
  • Ability to work with diverse communities in less familiar locations.

Equipment Operated:

La Red Health Center van, computers and video projectors, wide range of standard office equipment.

Work Environment:

Community outreach efforts in schools, businesses and churches. Outreach into areas where high risk populations reside and congregate. In most cases, controlled lighting, air quality, and temperature settings are consistent with internal office areas. Environmental hazards may be unpredictable including exposure to communicable diseases and biohazards.

Mental/Physical Requirements:

Involves standing, walking, kneeling, bending, grasping, manipulating and squatting and lifting (up to 50 pounds). Walking or standing may be more than four (4) hours per day. Must be able to work in a standard medical office setting, business or school. Ability to use a computer, and standard office equipment. Must have ability to read computer screens, printed materials and hearing and speech to communicate in person as well as over the phone. Must be able to attend meetings at various sites when required. Stress can be triggered by triggered by an individual’s response to workload or specific cases.
