Job Summary

The High Fidelity Wraparound (HFW) Case Manager is a critical role within CVS Health, responsible for safeguarding the principles of the wraparound process with the highest possible fidelity to the model. This position requires a strong understanding of the wraparound process and the ability to work collaboratively with various stakeholders to deliver high-quality services.

Key Responsibilities

  • Complete comprehensive strengths-based assessments and screenings of individuals, including Health Risk Questionnaires and Pediatric assessments.
  • Work in partnership with the High Fidelity team to develop and implement service plans, identifying providers of services or natural supports.
  • Facilitate High Fidelity Wraparound team meetings and conduct regular home visits to monitor services.
  • Identify high-risk factors and service needs that may impact member outcomes and care planning components, engaging clinical case management or crisis intervention as appropriate.
  • Coordinate and implement assigned care plan activities and monitor care plan progress.
  • Consult with case managers, supervisors, Medical Directors, and other health programs to overcome barriers to meeting goals and objectives.
  • Present cases at case conferences to obtain multidisciplinary review and achieve optimal outcomes.

Requirements

  • Reside in one of the following counties: Anderson, Boyle, Franklin, Jessamine, Marion, Mercer, Scott, Spencer, Washington, or Woodford, or within 50 miles outside of this required area.
  • 1+ years of experience providing Total Case Management services to children/youth with Severe Emotional Disturbance and their families.
  • 1+ year of experience or detailed knowledge of the Foster Care and juvenile justice systems, Adoption Assistance, the delivery of Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention Services, and evidence-based practices applicable to the Kentucky SKY populations.
  • Must be a Certified HFW facilitator or be willing and able to complete training and testing within 12 months of hire date.
  • Must be willing and able to travel 50% of the time in assigned region, with mileage reimbursed per company policy.

Education

  • Bachelor's Degree in Human Services or related field.

Compensation

The typical pay range for this role is $36.78 per hour. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors, including experience, education, geography, and other relevant factors. This position is eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range listed above.
