Job Description

We are also passionate about providing our employees the right tools to do their jobs effectively and provide the best hospice care in the Cleveland, OH area.Our team carries low patient caseloads. We take pride in our staffing levels and our ability to fulfill this promise for every hospice patient. What does this mean for our patients? It means more visits, better care, and the comfort that they deserve. By spending more quality time at bedside and communicating effectively, with patients and their families.

Shift information: 3 days per week, 7p-7a

What You'll Do:
  • Provide direct patient care as defined in State Nurse Practice Act
  • Implement current nursing practice following a comprehensive assessment and the plan of care
  • Provide accurate and timely documentation of patient services to reflect the plan of care
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
  • Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
  • Provide appropriate pain/symptom management. Evaluates the patient's response to treatments/medications

Qualifications:
  • Graduate of an accredited practical nurse or a vocational nursing program
  • Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
  • Currently licensed as an LPN/LVN in the practicing state
  • Must possess current CPR Certification
  • Demonstrates good verbal and written communication, and organization skills
  • Valid driver's license with an automobile that is insured in accordance with state requirements
  • Understands the philosophy of hospice concept and the needs of the terminally ill
