Rehabilitation Functional Manager

 

The Center for Elders’ Independence is a PACE (Program of All-Inclusive Care for the elderly) organization (PO) that uses an interdisciplinary team approach for care planning and implementing purposeful high quality, affordable, and integrated health care services to the elderly.  Our elderly meet PACE requirements as prescribed by CMS and are referred to as participants.  Our PO includes Adult Day Health Centers and primary care clinics, promoting participant autonomy, quality of life and the ability for individuals to live in their communities

 

THE POSITION:  Under the supervision of the Senior Director, Operational Excellence the Rehabilitation Functional Manager is responsible for the planning, implementing, and improving the PACE Rehabilitation services, with an emphasis on providing quality participant care. Supports the center operations in interviewing, hiring, and onboarding of Rehabilitation staff. Provides clinical leadership to train, and develop technical expertise of rehabilitation personnel that adds value and are aligned with organizational performance and quality measures. The Rehabilitation Functional Manager is responsible for the overall clinical functions focusing on effective and efficient operations of the Rehabilitation department to provide value-based care in the PACE setting.

 

DUTIES AND RESPONSIBILITIES:

  • Ensures Rehabilitation best clinical practices to uphold the highest quality of care and in compliance with the California Board of Speech-language pathology, Occupational Therapy and Physical Therapy Board of California, federal, state and local requirements.
  • Leads process improvement efforts with Rehabilitation Supervisors that support evidence-based practices and quality initiatives.
  • Utilizes quality improvement tools to collect, analyze, interpret, and display data that showed improved processes and participant care and reports during Quality Improvement meetings.
  • Collaborates with the Rehabilitation Supervisors and Center Leadership by responding and resolving participant care issues.
  • Collaborates with other disciplines as needed and acts as resource to Interdisciplinary teams across the program.
  • Participates in the talent acquisition process of Rehabilitation staff to ensure clinical competence, as needed by Center leadership.
  • Develops, implements, audits and communicates clinical practice, policies, and procedures for best practices, evidence based practice standards, and provides input to Rehab Supervisors on the evaluation of Rehab personnel at each PACE location.
  • Assesses and maintains a Rehabilitation staffing plan that efficiently and effectively utilizes the appropriate resources ensuring competency, skill mix and adequate number of staff.
  • Manages the clinical orientation and onboarding and competencies for Rehabilitation staff to ensure proper orientation and onboarding in a timely, appropriate, and comprehensive manner to meet all PACE program needs.
  • Provides functional and technical Rehabilitation consultation to Rehab Supervisors and Center Operations. Acts as subject matter expert in the development of performance improvement plans.
  • Maintains collaborative relationships with all stakeholders to improve service delivery and/or program growth.
  • Develop, improve, oversees training and monitor clinical workflows for efficiency, quality and satisfaction.
  • Acts as an administrative liaison and resource with administrative support departments and assists with regulatory management, including compliance in all areas of PACE Rehabilitation services including tools, equipment and participant safety, performance against national benchmarks and established service standards, and preparing the necessary Corrective Action Plan (CAP) as needed and in a timely manner.
  • Serves a leadership role in regulatory audits for both preparation and as a subject matter expert to ensure adherence to regulatory requirements.
  • Participates in developing, monitoring and revising  policies and procedures consistent with current standards of practice.
  • Determines activities tools/supplies and rehabilitation equipment needs and specifications consistent with program needs and budget in collaboration with center leadership.
  • Performs other related duties as assigned.

 

 

QUALIFICATIONS:

  • Graduate of an accredited program in Occupational, Physical Therapy or Speech Pathology or equivalent. (Masters in OT, Doctorate in PT- Preferred). Must a hold a current license in their respective discipline. (Required)
  • Minimum 5 years clinical experience in a geriatric setting working with frail elderly (ex. PACE, Acute Care, Skilled Nursing) (Required)
  • Experience in Interdisciplinary team environments is highly desirable. (Preferred)

 

  • Excellent clinical skills, as well as expert knowledge of PACE care delivery model and Interdisciplinary team function. Knowledge of relevant regulations from agencies such as OSHA, California Department of Healthcare Services (DCHS), Center for Medicare and Medicaid Services (CMS) – PACE Regulations.
  • Lean Six Sigma or equivalent process improvement certification (Preferred)
  • Ability to effect change in a highly matrixed organizational structure.
  • Ability to engage and influence individuals and teams to improve performance.
  • Ability to build and maintain effective communication and working relationships with internal and external customers (i.e. clinical team members, primary care providers, participants, caregivers/families, and center leadership teams).
