Job Summary

Key Responsibilities
  • Leadership and Clinical Oversight: Provide clinical leadership and oversight to our rehabilitation staff, ensuring they have the necessary skills and expertise to deliver high-quality care.
  • Process Improvement: Lead process improvement efforts to enhance our rehabilitation services, focusing on evidence-based practices and quality initiatives.
  • Quality Assurance: Utilize quality improvement tools to collect, analyze, and display data to demonstrate improved processes and participant care.
  • Collaboration and Communication: Collaborate with rehabilitation supervisors, center leadership, and other disciplines to ensure seamless care delivery and resolve participant care issues.
  • Talent Acquisition and Development: Participate in the talent acquisition process to ensure clinical competence and develop rehabilitation staff to meet program needs.
  • Clinical Orientation and Onboarding: Manage the clinical orientation and onboarding process for rehabilitation staff to ensure proper training and competency.
  • Regulatory Compliance: Ensure compliance with regulatory requirements, including OSHA, California Department of Healthcare Services, and Center for Medicare and Medicaid Services (CMS) - PACE Regulations.
  • Performance Improvement: Develop, implement, and monitor clinical workflows to improve efficiency, quality, and satisfaction.
  • Administrative Liaison: Serve as an administrative liaison and resource for regulatory management, including compliance with tools, equipment, and participant safety.
Requirements
  • Education: Graduate of an accredited program in Occupational, Physical Therapy, or Speech Pathology or equivalent.
  • Experience: Minimum 5 years of clinical experience in a geriatric setting, working with frail elderly (e.g., PACE, Acute Care, Skilled Nursing).
  • Certifications: Lean Six Sigma or equivalent process improvement certification (preferred).
  • Skills: Excellent clinical skills, expert knowledge of PACE care delivery model, and ability to engage and influence individuals and teams to improve performance.
