ChristianaCare is searching for a Practice Transformation Liaison to lead key initiatives within the ChristianaCare Clinical Alliance (CCCA) and CCHS: for example, closure of quality care gaps, workflow efficiency development, standardization and implementation of best practice, continuous learning, mentoring, and educating of population health strategies.

The successful candidate will have no less than five - seven years of work experience in ambulatory focused patient safety, quality or performance improvement.

In this role you will:

  • Develop recommendations for implementation based on analysis of clinical practice site workflows, staffing levels and procedures throughout CCHS.

  • Analyze, develop and conduct training needs for users of assigned clinical service lines as related to workflow. Conducts workshops and provides individual support for clinical operations as required including shadowing when appropriate. This position serves as a liaison/resource/advocate between clinical operations, IT, EHRs vendors, CCCA and other stakeholders to facilitate change.

  • Utilize Care Gap outcomes to promote benefits and availability of disease/case management services with providers and other health care providers. Identifies problems, develops process improvement strategies and assists the Care Gap team in Quality Improvement and reengineering activities. This includes identification and development of key clinical initiatives to improve quality and workflow to enhance clinic efficiency and patient engagement.

  • Other duties as assigned

Qualifications:

  • Bachelor's degree or other clinical degree, Operations, Business Administration, Healthcare Management, Statistics, or related field required. Master’s degree preferred.

  • Minimum of five years of work experience with at least two years in patient safety, quality or performance improvement

  • Minimum of 7 years’ related experience with at least 4 years’ ambulatory focused patient safety, quality or performance improvement may be considered in lieu of a Bachelor’s degree.

  • Organizational skills and project management required.

  • Knowledge of/or experience in any of the following is preferred: Outcome analysis, utilization management, clinical experiences in patient care processes, medical records, performance improvement, team facilitation and education (patient/staff) and physiology and disease processes.

  • Strong Excel skills preferred.

  • Knowledge of medical terminology and anatomy required.
