PRIMARY FUNCTION:

The Operations Director of Transitioning Programming for CareVio will have primary oversight of the acute space as it relates to case management, care coordination, and clinical integration of Population Health into the Accountable Care Organization and Clinically Integrated Network. This person is responsible for the direction and leadership of operational, programmatic, and personnel activities for the Transitional Care Team and services provided. This includes establishing, meeting, and continuously monitoring the goals and objectives of the program while maintaining alignment with collaborative partners. Fostering collaborative relationships which includes but are not limited to hospital systems, clinical care departments inclusive of emergency department, and ambulatory transitional providers such as home health, etc. will be instrumental to the success of this role. This person will be responsible for activities which drive successful outcomes while ensuring quality of care while meeting our contractual requirements. This position is responsible for advancing strategies to optimize the transition of patients across the continuum of care; working closely with facilities and clinicians to develop partnerships, coordinate clinical processes, and establish and evaluate metrics which drive success. The Operations Director of Transitional Programming will report directly to the Corporate Director of CareVio and in partnership with the Clinical Operations Director to assess program effectiveness associated with clinical and operational impact, enhance quality, improve cost, utilization, and patient, provider, as well as payer/stakeholder satisfaction.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Bachelor’s Degree in Science and/or Nursing (BSN) Required
  • Master’s Degree in health care related field required or successful achievement of a Master’s degree in health care related field required within 5 years of time in the position
  • 5-7 years of management experience in a health care setting
  • Advanced Certification in Leadership or Certification in Case Management or Case Management Administration required
  • An equivalent combination of education and experience may be substituted.

Physical Demands:

Extended periods of sitting with intermittent standing and walking.

Travel throughout the State of Delaware, MD, NJ, and other regional mid-Atlantic locations as required to collaborate with healthcare facilities across all CareVio locations
