Nurse Practitioner or Physician Assistant II - Nephrology

Responsibilities This position will be in the Nephrology & Hypertension division providing a combination of patient care in the clinic, hospital, and outpatient dialysis settings. Outpatient responsibilities include a NP/PA led chronic kidney disease clinic and collaborative subspecialty clinics, including outpatient dialysis rounds. The NP/PA inpatient responsibilities entail providing 24/7 consult coverage, managing chronic kidney disease (CKD), end stage renal disease (ESRD), acute kidney injury (AKI), hypertension, and acid base & electrolyte disturbances for patients throughout the hospital. The Nephrology & Hypertension NP/PA will work collaboratively with the multidisciplinary team, including nephrologists, fellows, nurses, and others. The role requires timely assessments, interventions, interpretation of diagnostic tests, and management of acute and chronic medical conditions. Patient and family education about recovery and disease process is also a crucial part of the health promotion for the patients. The NP or PA has advanced health assessment and decision-making skills, prepared through a formal educational program that meets criteria established by the profession. This individual works in collaboration with physicians and other health care professionals to provide health care across the continuum of care for assigned patients. The provision of care includes, but is not limited to: obtaining patient history, comprehensive assessment, ordering (including ordering dialysis), conducting, and interpreting diagnostic tests, prescribing pharmacological agents and treatments, and non-pharmacological therapies. Monitors the effectiveness of the therapeutic interventions. The ability to assess data reflective of the patient's status and appropriately interpret information relative to the patient's cultural and social economic status is required. The candidate demonstrates leadership through involvement on committees and team projects, education, and research and actively seeks to improve the practice through performance improvement initiatives. Must have excellent verbal and written communication skills and possess strong computer skills. Must be able to work collaboratively on a multi-disciplinary team. Must be flexible and willing to handle change. Position description: * Rotating shifts in hospital coverage and outpatient clinic visits. * Opportunities for specialization based on interests and practice needs, such as rounding in the outpatient dialysis unit, stone clinics, polycystic kidney disease clinics, etc. * Back up coverage for in-house overnight hospital coverage, as needed. * Work independently to the full scope of practice in compliance with state licensure. * Perform and documents in the medical record detailed patient history and physical examinations. * Coordinating daily rounds with the attending physician teams; presenting patient report to develop, collaborate and implement individualized plan of care. * Follows assigned patient population throughout the shift. * Deliver patient education on the spectrum of CKD care and renal replacement therapies, including dialysis modality options. * Consult with surgical teams to promote optimal dialysis access placements. * Coordinate care of ESRD patients with outpatient dialysis centers from admission to discharge. * Prescribe appropriate pharmacological treatments and therapeutic procedures, including dialysis and continuous renal replacement therapies, as per accepted evidence-based guidelines and expected institutional protocols. * Monitor effectiveness of therapeutic interventions. * Participate in the delivery of Tele-Health. * Make appropriate referrals as needed. * Examine plan of care for cost-effectiveness and achievement of expected outcomes. * Hospital coverage includes: * Assist with ongoing departmental process improvement and policy development. * Actively participates in grand rounds, quality improvement, and faculty meetings. * Precept, mentor and instruct students, graduates, and novice NP/PAs. The complete interview will be reviewed by a Mayo Clinic staff member and you will be notified of next steps. Qualifications Nurse Practitioner: Masters-prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current FL RN and APRN license and certification as a FNP, ANP, ACNP. Physician Assistant: Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the FL State Board of Medical Practice. Preferred Qualifications: * >1 year experience, new graduates may apply. * Prior training and education in nephrology, hemodialysis, peritoneal dialysis, and kidney transplant. * Experience providing care in patient's homes or via telehealth/remote monitoring. Additional Qualifications: BLS, ACLS certification from American Heart Association or American Red Cross and DEA registration required. * Flexibility to adjust schedule to meet patient and staff needs. * Excellent verbal and written communication skills. * Experience with Microsoft 365 and Teams * Strong computer skills in an Electronic Medical Record.
