Position Overview:

Faith Home Health & Hospice is in search of empathetic and skilled Licensed Practical Nurses (LPN) committed to delivering exceptional care and professionalism.

As a Home Health LPN, you will be responsible for providing individualized nursing care to patients, following a detailed care plan established by the RN Case Manager. This plan outlines specific goals based on nursing assessments and includes preventive and rehabilitative nursing interventions.

You will collaborate with a dedicated team of healthcare professionals to ensure comprehensive patient care.

We are seeking Full-Time or PRN Licensed Practical Nurses (LPN) in the Overland Park, KS area. Details regarding specific coverage areas will be discussed during the interview.

Key Responsibilities:
  • Deliver skilled nursing care in patients' homes or facilities as per the treatment plan.
  • Evaluate patients' conditions and maintain accurate documentation of their health status.
  • Educate patients, families, and caregivers while considering individual needs and care standards.
  • Perform wound care, manage diseases, and conduct medication reviews as directed by the physician and supervising RN.
  • Collaborate effectively with patients, physicians, and the interdisciplinary team.

Qualifications:
  • Valid and unencumbered LPN license from the Kansas and Missouri State Board of Nursing (compact or multi-state license preferred).
  • Minimum of 1 year of experience in home health.
  • Possess a valid driver's license, reliable transportation, and auto insurance.
  • Current CPR certification or willingness to obtain.

Job Type: Full-time
Compensation: Starting from $58,000.00 annually
