*LPN Responsibilities: *
* As a Home Health or Hospice Licensed Practical Nurse (LPN), you will be responsible for but not limited to:
* Providing skilled nursing in the home or facility as ordered on the plan of treatment and nursing plan of care.
* Assessing patients' condition, communicating, and properly documenting any pertinent information about the patients conditions.
* Promoting patient/family/caregiver education taking into account the patient's needs and the standards of care.
* Responsible for performing wound care, disease management, and medication reviews in the home setting as directed by physician and supervising RN.
* Functioning as a team member by collaborating with the patient, physician and interdisciplinary team.
LPN QUALIFICATIONS:
* Have current, valid unencumbered Kansas and Missouri State Board of Nursing LPN license (compact or multi-state license preferred).
* Have a minimum of 1 year of home health experience.
* Possess a valid driver’s license, reliable transportation, and auto Insurance.
* Have or obtain a current CPR certification.
