Description

POSITION SUMMARY: The Palliative Care Nurse Coordinator functions independently with licensed providers to plan and deliver patient care for Palliative Care patients across the continuum of inpatient and outpatient care, ensuring compliance and maintain a high standard of nursing care. Under the primary supervision of the Palliative Care Medical Director or Provider, will develop skills in and share responsibility for assessing, developing, implementing and evaluating the holistic care of the patient and their family facing serious illness and end of life.  The Nurse Coordinator will carry out duties independently, while also working skillfully in a collaborative role throughout the organization, with patients and families, providers, case managers, and all relevant disciplines.  The Palliative Care Nurse Coordinator will assist with the discharge process, arrange follow up care, complete medication authorizations, assist patients with barriers to care and overall be an advocate for the patient. The Palliative Care Nurse Coordinator will work closely with the hospitalist service and case management team.

This position requires the full understanding and active participation in fulfilling the Mission and Core Values of CHRISTUS St. Vincent. He/She shall support CHRISTUS St. Vincent’s strategic plan and the goals of the Palliative Care Program.



Requirements

MINIMUM QUALIFICATIONS:

EDUCATION:  Graduate of an accredited program for Registered Nursing. BSN preferred.

CERTIFICATION/LICENSES: Current New Mexico RN license or current Compact state license.  BLS certification required. ELNEC training, or equivalent, preferred.  Certification in Hospice and Palliative Care required within one (1) year of hire if currently not certified.

SKILLS:

  • Ability to provide age specific, quality and patient-centered care to all patients through the clinical process and manifest standards of practice with sensitivity and respect for the diversity of human experience
  • Knowledge of current Palliative Care principles, practices, standards, and approaches in the health care profession, and the health care system, and the responsibility and accountability for the outcome of practice.
  • Proven ability to communicate effectively with staff that has a variety of levels of knowledge, skills and responsibilities
  • Professional work habits and demeanor, including
  • Ability to work independently or with limited supervision
  • Ability to work well with people from different disciplines who may have varying degrees of technical expertise
  • Ability to work in a team environment and to contribute to the effective functioning of that team
  • Ability to discuss goals of care with patient/family and team
  • Knowledge of NM Advanced Directives, including completing, changing and communicating AD’s, and the skills to participate in the Advanced Care Planning activities of the hospital
  • Excellent communication skills using adult education principles to educate patients, families and hospital staff
  • Maintain up-to-date knowledge around the availability of community resources in order to assist with the discharge process
  • Will provide primary education and external referral support, in collaboration with Case Management, when patients are referred to hospice.
    • Medicare rules and regulations pertaining to hospice care at home, GIP hospice care in the hospital, and be familiar with all the hospice benefit levels.
    • Medicare rules and regulations pertaining to home health care, for non-hospice patients.
  • Strong Computer Skills.
  • Effective organizational skills

EXPERIENCE:  5 years nursing experience, two years of which must have been in hospice or palliative care.

NATURE OF SUPERVISION:

Exposure to infectious diseases and x-rays.

PHYSICAL REQUIREMENTS: Must be able to handle emergency/crisis situations, prolonged, extensive or considerable standing and walking. Works in a clean, well-lighted, ventilated smoke-free environment.  Subject to stressful professional relationships.  Working hours are varied, with flexibility due to unexpected changes in schedule and emergencies.
