Summary: The Community Care Coordinator will provide support, education, and assistance to address treatment and ongoing needs. They will identify obstacles to successful treatment and assist family to reduce these barriers via referrals, advocacy, linkages to community supports, and creative problem solving. The position may also include general community education and support services. Prepares requisitions ordering laboratory tests, x-ray exams, blood tests, etc. and scheduling appointments. Performs case data entry in the EMR system. Schedules and coordinates dates between patients, physicians, and hospital, if applicable. Schedules other diagnostic testing when necessary.

Education: High School graduate or GED required. Bachelor's degree social services / human services or case management certification preferred.

Licensure: None

Experience: Community care work experience. Experience in Human Services.

Skills and Abilities: Bilingual a plus. Ability to communicate with a variety of people. Ability to utilize sound interpersonal skills when interacting and taking into consideration cultural background, linguistic skills and age of customers.

· Organizational, coordinating, and advocacy skills.

· Skill in exercising initiative, judgment, problem-solving, decision making, and multi-tasking.

· Skill in developing and maintaining effective relationships with medical and administrative staff and the public

· Ability to work comfortably with families/ patients of diverse backgrounds

· Communication and record keeping skills

· Maintains confidential information (HIPAA).

· Demonstrated customer service skills.

· Ability to utilize responsive customer service skills when interacting and taking into consideration cultural background, linguistic skills and age of customers.

· Ability to work cooperatively with peers and other disciplines.

· Ability to communicate effectively and work collaboratively with peers and other interdisciplinary teams.

· Skills in organization and priority setting.

· Working knowledge/competence with state and national standards (i.e., PCMH and Department of Health).

· Professional accountability, motivation and self-direction, positive attitude & team player
