Title Registered Nurse RN Case Manager, Hospice - The RN Case Manager plans, organizes, directs and provides hospice care to meet the identified needs of patients and families. The RN provides assessments of the psychosocial and physical status and also initiates and develops a plan of care in collaboration with the Interdisciplinary group. 

Hours:  Full-time |  Days |  Monday - Friday with on call rotation.

Pay:  Annual Salary Range:  $81,960 - $94,440 dependent upon years of experience plus additional On Call pay.

Territory: You will have a general territory within our service area where you will provide in-home care to our patients wherever they live (personal home, nursing home, etc.).  The territory for this position would primarily be in the South section of the Twin Cities Metro area.

Minimum Requirements:

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of MN.  License is active and in good standing.
  • Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.
  • If a new graduate, 1200 verified hours of employment as a graduate or registered nurse within the last twelve (12) months or evidence of completion of a company-approved Preceptor Program.
  • One (1) year of hospice care experience preferred.
  • Proof of valid driver’s license.
  • Proof of valid auto liability insurance.
