Role Description

This is a full-time role for an RN Case Manager at Hamilton Hospice. The RN Case Manager will be responsible for discharge planning, utilization management, nursing, medicine, and case management. This role will be primarily based in Dalton, GA, with flexibility for some remote work.


Qualifications

  • Discharge Planning, Utilization Management, and Case Management skills
  • Strong background in Nursing and Medicine
  • Active Registered Nurse (RN) license
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Experience in hospice or palliative care is a plus
  • Associate’s or bachelor’s degree in Nursing or related field
